Grievance and Appeal

As a Family Care member, you share and have responsibility in the decision-making process with your Team to help achieve the best overall treatment and support. Although you are a full partner in developing your Member Centered Plan/care plan, there will be times when there is disagreement on which services are necessary or reasonable. For this reason, ContinuUs has a grievance and appeal system in place

You have the right to file a grievance or appeal a decision made by ContinuUs and to receive a prompt and fair review. You will not get into trouble if you complain or disagree with ContinuUs. We won’t treat you differently if you file a grievance or appeal with ContinuUs, our providers, or the State of Wisconsin. We want you to be satisfied with your care.

You can:

 

Family Care has multiple mechanisms in place to address your concerns.
You can use any or all of these methods together or at different times.
However, each way has different rules, procedures and timelines.

 

If you have a particular type of concern that you do not know how to resolve, you can ask your Team or the Member Rights Specialist any time you need help with a grievance or appeal, or have questions about your rights. Ombudsman and advocates are available to help all Family Care members with complaints, grievances, and appeals. You can also be assisted by a family member, friend, attorney, or any other person willing to help. Our Member Rights Specialist may be able to give you information about other places that can help you too.
