At ContinuUs, we help members manage their long-term care through a process of collaborative stewardship. By joining our team, you become an integral part of that effort by sharing your talents and dedication to the mission of ContinuUs, a leader in managed care for frail elders and the physically and developmentally disabled.

Throughout our territory, Inter-Disciplinary Teams (IDTs) made up of a Care Manager, Nurse Care Manager, the member, and any family or other supportive persons in their lives actively engage in planning to meet members’ clinical, functional, and personal outcomes through individualized care plans that are sustainable and designed within a financial framework.

Other roles at ContinuUs make the work of the IDTs possible through their own professional contributions, be they managerial, administrative, technical, financial or quality-oriented.

How We Work

 

Mission Statement

Through a network of service providers, ContinuUs offers long-term care support to our Members – adults with disabilities or frailties who enroll in Wisconsin’s Family Care program.

We work together with our Members, the families of our Members, our service providers, and many others to:

  • Match the needs of our Members with appropriate, cost-effective long-term care services;
  • Support Members to be in control of their own lives;
  • And help our Members live as independently as possible, for as long as possible, in the community of their own choosing.

We pursue this mission out of a passion to help those with compelling needs, to enrich the communities in which we all live, and to be good stewards of public funds.
